Steps to Care

Joining the Coastal Care Family


With varying personal circumstances dictating change and emotions often playing a role, we understand that choosing a private caregiver is a big decision. Coastal Care aims to put you at ease by offering:

Step 1 - A Complimentary Consultation

To best serve you, we need to get to know you. A Coastal Care nurse will initiate our relationship by paying a visit to your home for a complimentary consultation. For no fee, we will take the time to discover your needs and wishes for your private care. Your family’s wishes are of importance because family is key to your optimal health and happiness. The Coastal Care nurse will also learn about your personality at this visit so we can best match you with a compatible caregiver.

Step 2 - Personalized Care Plan

After your complete in-home assessment, Coastal Care creates a personalized plan of care to meet your unique needs and requests. Together, the client and their family decide on visit times, frequency and scope. For example, you may choose a caregiver to visit for a few hours in the morning. Your carefully matched caregiver will arrive on time, gently assess where you are in your day and move forward with your outlined plan of care. Whether your plan includes daily, weekly or monthly visits, Coastal Care Nursing offers many services (some surprising!) to private care clients.

To further exceed your expectations and satisfaction, Coastal Care assigns an LPN or RN to you at no additional cost. Each Coastal Care client plan includes a quarterly visit from one of these nursing professionals to reassess your needs and update your plan of care.

Step 3 - Matchmaking

Your personalized plan of care also includes a carefully selected caregiver, from someone who simply lends an ear or a helping hand, to someone who can provide specific medical expertise/capabilities. In fact, Coastal Care takes great pride in its matchmaking because a perfect pairing allows you and your family to develop both trust and a personal relationship with your caregiver. This means our professional caregivers can better serve you through true continuity of care, which is especially important for those experiencing cognitive impairment.

Step 4 - Reassurance Guarantee

Finally, each Coastal Care Nursing client is assigned a Private Care Coordinator who ensures the caregiver relationship is running smoothly. This trusted professional makes Weekly Reassurance Calls to both clients and their families to make sure everyone is satisfied with our service. Our Private Care Coordinators also handle schedule changes when necessary, work with your assigned LPN/RN to routinely assess your needs, and act as resources for clients who need more information or guidance on such topics as Alzheimer’s and Dementia.

In addition to these benefits, Coastal Care can also relieve you and your family of the burden of handling long-term care insurance.

Step 5 - Coastal Care Nursing Transitional Services

If you’re not sure if a private caregiver is the right choice for you, Coastal Care offers Transitional Services to help with the decision-making process.

Our Transitional Services will provide all the details necessary to decide between staffing private care at home or moving into a residence at an assisted living facility. In fact, you could do both. Many clients utilize our private caregiver services while waiting for their new residence to become available. Some then choose to continue to receive Coastal Care’s services after moving because of the close relationship they have developed with their caregiver.
